29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about October 12, 2019, an employees were exposed to caught-in hazards while cleaning the shaft of a bone collector installed on the South Wolfking Grinder in the Blending Department in Plant 2. The employer did not ensure guarding was installed to eliminate amputation hazards such as those created by the point of operation. There is no abatement certification or documentation required for this item.

29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure and post exposure evaluation and follow-up to all employees who had an exposure incident: On or about October 12, 2019, employees received an occupational exposure to blood and other potentially infectious materials (OPIM) while assisting an employee and cleaning droplets of blood following a work-related injury on the South Wolfking Grinder in the Blending Department inside Plant 2. The employer did not ensure that the Hepatitis B vaccine and vaccination series were made available and offered to the employees with occupational exposures. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

The employer did not ensure employees were trained prior to their initial assignments where an occupation exposure would take place. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
