Paper Print Department - On or about January 16, 2020, a maintenance worker performed service and maintenance by installing safety gates on Press 6.  The employer failed to conduct a periodic inspection of the energy control procedure, at least annually, as required by the standard.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:      a) Paper Print Department - On or about January 8, 2020, the employer failed to provide adequate training for employees required to perform service and/or maintenance activities on the line 6 press.   A machine operator attempted to brush dry ink flakes from the print plate while performing an ink quality check and sustained four crushed fingers and the amputation of four fingertips when his right glove was pulled into the anilox roller.  The machine was not de-energized and locked out while the employee was performing this task.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     a) Thermoforming Department - On or about October 18, 2019, machine operators were exposed to machine hazards associated with moving parts and in-going nip points during set-up on the Line 8 Thermoforming Machine when threading a starter/feeder sheet. The employer failed to implement energy control application steps as the machine was not shut down or turn off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.    b) Paper Print Department - On or about January 8, 2020, machine operators were exposed to machine hazards associated with moving parts and in-going nip points when performing set-up, ink print checks and cleaning ink flakes from the print plate on the Line 6 Press. The employer failed to implement energy control application steps as the machine was not shut down or turn off to perform the servicing work [per the 1910.147(d)(2) requirements]. 29 CFR 1910.1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:    Dart Container Corporation of Illinois dba Dart Container Corporation - Chicago, IL - On or about October 23, 2019, the employer failed to record the following workplace injuries in the detail required by the regulation on the OSHA 300 Log for calendar year 2019.     a) On or about October 18, 2019, a recordable injury occurred when a machine operator was hospitalized after sustaining a crushed right arm while threading the starter/feeder sheet on the Line 8 Thermoforming Machine, resulting in lost days away from work.  The employer failed to complete Columns E and F of the OSHA 300 log to describe the location of the accident, part of the body affected, and the object that caused the injury.
