Thermoforming, Line 5 - On or about February 9, 2022, the employer did not guard the point of operation to prevent employee(s) from having any part of their body in the danger zone during the operating cycle on the trim press machine, thereby exposing employees to amputation hazards. An employee sustained a left middle fingertip amputation while removing a lid from the line 5 trim press machine chute.
