29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:  Grey Eagle Construction LLC worksite located at 4002 McCallister Dr., Perrysburg, Ohio 43551:  On or about October 24, 2019, the employer did not maintain accident prevention programs to ensure compliance with common construction industry hazards, such as fall hazards, ladders use, and personal protective equipment.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:   Grey Eagle Construction LLC worksite located at 4002 McCallister Dr., Perrysburg, Ohio 43551:  On or about October 24, 2019, the employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing framing operations from elevated surfaces greater than six feet from the level below.

Grey Eagle Construction LLC worksite located at 4002 McCallister Dr., Perrysburg, Ohio 43551:  On or about October 24, 2019, the employer did not ensure employees were protected from eye injuries while installing gable studs on single story pool house structure using pneumatic nailers.

Grey Eagle Construction LLC worksite located at 4002 McCallister Dr., Perrysburg, Ohio 43551:  On or about October 24, 2019, the employer did not ensure employees were protected from falls while installing gable studs on a single story pool house structure. The employees were exposed to a fall distance of approximately 9 feet from a steep slope roof.
