29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:   A)     Grey Eagle Construction, LLC, conducting work at 15594 John F. McCarthy Way, Perrysburg, OH 43551: On or about July 1, 2020, the employer did not ensure that employees did not stand on or above the top two rungs/steps of the 6-foot Werner stepladder. Employees used the ladder to access the roof of the one story residential structure.  Employees were exposed to approximately 6 feet fall hazard due to the improper use of the ladder.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:  A)     Grey Eagle Construction, LLC, conducting work at 15594 John F. McCarthy Way, Perrysburg, OH 43551: On or about July 1, 2020, the employer did not have a plan or program to identify job site hazards, which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during job site operations.    Grey Eagle Construction, LLC was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.20(b)(1)), which was contained in OSHA Inspection # 1440089, Citation number 1, Item number 1A, and was affirmed as a final order on 12/05/2019, with respect to a workplace located at 4002 McCallister Drive, Perrysburg, OH 43551.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  A)     Grey Eagle Construction, LLC, conducting work at 15594 John F. McCarthy Way, Perrysburg, OH 43551: On or about July 1, 2020, the employer did not ensure that employees were adequately trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while conducting framing activities atop the one story residential structure. Employees were exposed to approximately 11 feet fall hazard from the structure to the ground below.    Grey Eagle Construction, LLC was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.503(a)(1)), which was contained in OSHA Inspection # 1440089, Citation number 1, Item number 1B, and was affirmed as a final order on 12/05/2019, with respect to a workplace located at 4002 McCallister Drive, Perrysburg, OH 43551.

McCarthy Way, Perrysburg, OH 43551: On or about July 1, 2020, the employer did not ensure that employees were adequately protected from fall hazards while conducting framing activities atop the one story residential structure. Employees were exposed to approximately 11 feet fall hazard from the structure to the ground below.
