McCarthy Way, Perrysburg, OH 43551: On or about July 1, 2020, the employer did not ensure that employees did not stand on or above the top two rungs/steps of the 6-foot Werner stepladder. Employees used the ladder to access the roof of the one story residential structure. Employees were exposed to approximately 6 feet fall hazard due to the improper use of the ladder.
