29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:        a) On or about October 24, 2019, employees were exposed to eye injury while operating a pneumatic nailer to fasten rafters on a second story/attic deck without eye protection.     b) On or about October 24, 2019, employees were exposed to eye injury while operating an electric saw to cut lumber on the ground level without eye protection.        In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:           a) On or about October 24, 2019, employees were exposed to electrical hazards where carpentry work was conducted adjacent to 120/240 volt, 200 amp electrical powerline service drop.       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):                      a) On or about November 24, 2019, employees were exposed to the hazard of falling approximately 20 feet to the exterior of the building while working from the top plate and deck of the second story/attic without the benefit of fall protection.                   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
