29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On December 13, 2022, at the above addressed jobsite, employees utilized an extension ladder which did not extend three (3) feet above the upper landing being accessed.  No abatement certification or documentation is required for this item.

29 CFR  1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) On December 13, 2022, at the above addressed jobsite, employees utilized an extension ladder with the feet placed in elevated position and were not in contact with the floor to ensure ladder would not slide out from employee when ascending and descending.  The ladder not secured in any other fashion.  No abatement certification or documentation is required for this item.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On December 13, 2022, at the above addressed jobsite, employees performing sheeting of the second story of a new construction residential home were exposed to fall hazards of approximately 11 feet.  No abatement certification or documentation is required for this item.    Prime Builders Construction Group, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1440510, citation number 1, item number 3, and was affirmed as a final order on December 26, 2019, with respect to a workplace located at 209 4th Street in Wilmette, Illinois.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   a) On December 13, 2022, at the above addressed jobsite, employees were exposed to eye injury while operating a pneumatic nail gun without eye protection during carpentry work.  b) On or about December 13, 2022, at the above addressed jobsite, employees were exposed to eye injury while operating a circular saw without eye protection during carpentry work.  No abatement certification or documentation is required for this item.
