29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) DICK'S SPORTING GOODS, 3595 CAPITAL CITY DRIVE, CAMP HILL PA - An employee sustained an amputation injury on August 24, 2019, 2:15 pm and the employer did not report the amputation injury to OSHA until August 26, 2019 10:02 am. No abatement certification or documentation required. Dick's Sporting Goods was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1288044, citation number 1, item number 1, and was affirmed as a final order on February 15, 2019, with respect to a workplace located at 8001 South Orange Blossom Trail, Orlando, FL 32803.
