29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective. On or about October 21, 2019 the Solid Mud Man's Trailer was not properly grounded to earth. This condition exposed the employees to electrical shock hazards.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were run through doorways, windows or floors. On or about October 21, 2019, at the Water & Sewage trailer a flexible electrical cord (AWG12 50 ft. orange cord) was pinched between a heavy metal door. Employees were exposed to the hazard of electrical shock.
