29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets:  a) Jobsite, 1000 Nevillewood Drive, Nevillewood, PA,  On or about October 29, 2019, four employees were working on a flat roof and were wearing fall protection, but not utilizing the required methods of utilizing appropriate anchorage points and exceeding the permissible fall distance.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite, 1000 Nevillewood Drive, Presto, PA:  On or about October 29, 2019, four employees were working on a flat roof and were wearing fall protection, but not utilizing the required methods to protect the employees.  The employer told the csho that he has the fall protection training records for the employees and would send it, but the csho never received it.
