29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on or about October 30, 2019 when two employees installing dry in and metal flashing on a one-story, 5:12 pitch garage are not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems, exposing employees to 10-foot fall hazard.    DDR Quality Roofing & Sheetmetal, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(C), which was contained in OSHA inspection number 1327745, citation number 1, item number 1 and was affirmed as a final order on August 8, 2018, with respect to a workplace located at 158 Cerise Circle, Lot 206, Daytona Beach, FL 21124.      DDR Roofing, LLC (Previous company name with same owner), was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(D)(16), which was contained in OSHA inspection number 1278939, citation number 1, item number 1 and was affirmed as a final order on January 2, 2018, with respect to a workplace located at 3889 S. Atlantic Ave, Daytona Beach, FL 32118.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.     a. On or about October 30, 2019 the fiberglass extension ladder used by employees to access the roof was not secured to prevent displacement, exposing employees to a fall hazard.
