29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On the east side of the construction site, employees engaged in decking work used pneumatic nail guns and were not wearing eye and face protection.  The employees were exposed to struck-by hazards and eye injuries.

29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw.   On the east side of the construction site, employees operating a Makita skill saw with the lower guard wedged in the up position while cutting decking material were exposed to amputation hazards.

On the east side of the construction site, employees engaged in decking work without fall protection, were exposed to fall hazards of approximately 9-feet to the ground below.

On the east side of the construction site, the employer did not provide an extension ladder where employees could access and egress the building's upper landings, exposing employees to potential fall hazards.
