29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: Instance 1 at 2nd floor: On or about 10/29/19 and at times prior, the employer exposed employees fall hazards, in that guardrails were not installed in the inner portion of the stair tower facing the building, which employees climbed up the stair tower to access the upper floors. Instance 2 at 3nd floor: On or about 10/29/19 and at times prior, the employer exposed employees fall hazards, in that guardrails were not installed in the inner portion of the stair tower facing the building, which employees climbed up the stair tower to access the upper floors. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Instance 1 at Bldg 6: On or about 10/29/19 and at times prior, the employer exposed an employee to a fall hazard of approximately 26ft due to not ensuring an employee attached his lanyard to a safety rope while sandblasting at the edge of the 3rd floor balcony. Instance 1 at Bldg 7: On or about 10/29/19 and at times prior, the employer exposed employees to a fall hazard of approximately 9ft due to lack of guardrails at the edge of the balcony while employees were storing building material close to the edge of the balcony.
