29 CFR 1910.147(f)(1): The sequence of actions required by 29 CFR 1910.147(f)(1) when testing or positioning of machines, where temporary removal of lockout or tagout devices from the energy isolating device is required, were not followed. a) Tile Cutting Department - Schon & CIE GMBH /C90 Tile Cutting Press machine : On or about November 1, 2019, the employer exposed employees to struck-by and caught-in hazards, in that testing or positioning of machine for temporary removal of the lockout/ tagout devices were not followed for the Schon & CIE GMBH /C90 Tile Cutting Press machine (Serial # 60339/64 & Machine Type: 2050) while employees were replacing chain for the machine.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a) Tile Cutting Department - Schon & CIE GMBH /C90 Tile Cutting Press machine : On or about November 1, 2019, and at times prior, the employer exposed employees to struck-by and caught-in hazards, in that the employer did not provide training to employees to ensure that the purpose and function of the energy control program were understood when employees were replacing the chain for the Schon & CIE GMBH /C90 Tile Cutting Press machine (Serial # 60339/64 & Machine Type: 2050). Repeat - Engineered Floors, LLC. was previously cited for a serious violation of the same occupational safety and health standard and hazard of 29 CFR 1910.147(c)(7)(i), which was contained in OSHA inspection # 1349568, Citation 1, Item 2 and was affirmed as final order on 3/14/2019, with respect to a workplace located at 3201 N. Bypass, Dalton, GA 30720.
