29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  AFSM Machine 3: The employer did not ensure the flexible cord entering the housing point was provided with strain relief.  Employees conducted preventive maintenance checks and services on the AFSM machine and were in the immediate area of the electrical hazard.  Employees were exposed to electrical shock due to the strain relief (rubber sheathing) being pulled away from the coated electrical wire plug housing point.
