29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  Employees were exposed to fall hazards.  Employees engaged in maintenance activities using the Genie, Model Z-4525, Serial number 0474 were exposed to fall hazards greater than four feet from the work platform to the concrete floor below.  Employees were exposed in that no personal fall protection/restraint system was utilized.

Employees engaged in maintenance activities using the All Ball Bearing bench grinder, model #60650, Serial #G, in the maintenance shop were exposed in that the grinder was not equipped with a work rest.

29 CFR 1910.215(b)(3): Bench and floor stands. The angular exposure of the grinding wheel periphery and sides for safety guards used on machines known as bench and floor stands did exceed 90 degrees or one-fourth of the periphery.  This exposure did begin at a point not more than 65 degrees above the horizontal plane of the wheel spindle:  Employees were exposed to struck-by hazards.  Employees engaged in maintenance  activities in the machine shop at the All Ball Bearing bench grinder, Model number 60650, serial number G, were exposed to struck-by hazards in that a machine tongue guard was not provided.
