The employees work where exit directional signs are not posted. On 11/04/2019 employees worked in aisles such as, but not limited to, 6600 where the nearest exits were not immediately apparent. Exit directional signs were not posted to prevent employee exposure to fire hazards.

The employees are permitted to walk in unmarked aisles used for material handling equipment. On 11/04/2019 employees walked in permanent aisles which were not marked. The aisles were also used by material handling equipment such as, but not limited to, forklifts.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service: personnel: Warehouse: Employer is exposing employees to struck-by hazards. The employees are permitted to operate defective powered industrial trucks. On 11/04/2019 employees operated powered industrial trucks which had defects such as, but not limited to, faulty braking systems, erratic lifting operations, and badly damaged tires (Crowne SP 3000 (FCA 99)). The status of the tires had been marked as "repairs needed" by a servicing vendor contracted to provide maintenance activities for the powered industrial trucks at the establishment. The equipment was not removed from service until the defects were corrected.

Employees are permitted to operate unguarded drill press. Employees were exposed to amputation hazards where they used a Westward 15" Drill Press which did not have a guard for the rotating edges of the chuck and drill bit.
