29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Worksite located at 1516 NE 96th St., Liberty, Missouri- An elevated employee was exposed to fall and struck-by hazards in that the elevated employee was raised into position and then lowered down off from being elevated by an employee that had no formal training on safe forklift usage.
