On the NE wall of the Maintenance Leads Office, where employees were exposed to electrical shock hazards when an electrical cord was wired into the terminal ends of a 240-VAC knife switch and used as a substitute for fixed wiring.     b. Inside the Maintenance Leads Office on the north wall, where employees were exposed to electrical shock hazards when an electrical cord was installed inside a 110-VAC wall outlet and used as a substitute for fixed wiring. .
