29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On 10/18/2019, employees of Jesus Rodriguez were working on a residential building located at 3020 Elm Street, Saint Charles, MO.  Employees were using a pneumatic roofing nail guns without the use of eye protection.

29 CFR 1926.300(b)(2):  Moving Part(s) of equipment exposed to contact by employees or which otherwise created a greater hazard, were not guarded:  On 10/18/2019, employees of Jesus Rodriguez were performing roofing operations at 3020 Elm Street, Saint Charles, MO.  Employees were exposed to caught in-between hazards while operating a Jenny Products air compressor, Model# C6HGA-8P, Serial # G062475009.  The guard protecting the starter motor was removed, while in use.

29 CFR 1926.416(a)(1):  Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  On 10/18/2019, employees of Jesus Rodriguez are exposed to electrical hazards while performing roofing operations at 3020 Elm Street, Saint Charles, MO.  Employees were performing work near unprotected energized service drop.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 10/18/2019, employees of Jesus Rodriguez were performing roofing operations at 3020 Elm Street, Saint Charles, MO.  Employees were installing shingles approximately 8 feet above a lower level.  Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.  On 10/18/2019, employees of Jesus Rodriguez were performing roofing operations at 3020 Elm Street, Saint Charles, MO.  The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.
