29 CFR 1910.29(b)(5): The employer failed to ensure that midrails, screens, mesh, intermediate vertical members, solid panels, and other equivalent intermediate members are capable of withstanding, without failure, a force of at least 150 pounds (667 N) applied in any downward or outward direction at any point along the intermediate member: Location: Shredder facility, 69 Rover Street, Everett, MA a) On or about November 5, 2019, an employee conducting a repair to a damaged midrail and corner post fell through the railing to the surface below, an approximate 24'9" fall, when the midrail broke loose. The damaged midrail was not capable of withstanding a force of at least 150 pounds.

29 CFR 1910.179(j)(3): Complete inspections of the crane was not being performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section: Shredder facility, 69 Rover Street, Everett, MA a) On or about November 5, 2019, periodic inspections of cranes did not include a physical inspection of the crane. By not physically inspecting the crane, problematic conditions can not be detected before causing potential problems.
