29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about September 9, 2019 an employee engaged in wooden truss building operations was operating Floor Truss Table #8 when a pneumatic clamp was engaged while the employees left hand was still positioned on the wooden truss, exposing employee to the hazard of being pinched by floor truss clamps.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: On or about November 6, 2019 and at times prior thereto, power-transmission conveyor roller's had unguarded projecting shaft ends that varied in lengths that were 1.5 times the length of the shaft ends diameter, exposing employee's to the hazard of getting caught in conveyor rollers with projecting shaft ends.
