IIAR 2 - 2012, the employers chosen RAGAGEP, Section 11.3.6.3 requires that the extremity of the pressure relief device(s) discharge piping relieved to atmosphere shall be 20 ft. or more from any window ventilation intake, or personnel exit, or as specified by the jurisdictional authority.    (b) Location - 330 Lynnway, Lynn, MA:    On or about 11/07/2019, the employer did not document that its ammonia refrigeration equipment complied with recognized and generally accepted good engineering practices (RAGAGEP), such as IIAR 2, when the discharge from pressure relief devices to the atmosphere was not arranged to avoid spraying ammonia on persons in the vicinity. IIAR 2 - 2012, the employers chosen RAGAGEP, Section 11.3.6.4 requires that the discharge from pressure relief devices to the atmosphere shall be arranged to avoid spraying of refrigerant on persons in the vicinity.      (d) Location - 330 Lynnway, Lynn, MA:    On or about 11/07/2019, the employer did not document that its ammonia refrigeration equipment complied with recognized and generally accepted good engineering practices (RAGAGEP), such as IIAR Bulletin No. 114, when insulated piping on the roof was not marked with a label before and after a transition from vertical to horizontal from a lower level of the roof to an upper level of the roof. IIAR Bulletin No. 114 - 1991, the employers chosen RAGAGEP, Section 5.0 requires that piping markers shall be located before and after a change in piping direction.
