29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (1) authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. (2) turn off and shut down the machine (3) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy sources. (4) affix lockout or tagout devices to each energy isolating device (5) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergizing of the machine or equipment have been accomplished. a) On 11/8/2019 an employee opened an inspection door on a Rag Tearing Line Section #2 (model # M 1221) to perform servicing/maintenance and was not required to lock out the machine prior to beginning the jam clearing process.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On or about 11/8/2019 an employee was operating a Rag Tear Line during production. Raw material had become lodged in the rollers of Section #2 (Model 1221). The employee opened an inspection door, (that was not adequately guarded to prevent movement of the rollers) to remove the excess raw material from the rollers, the employees hand was pulled into the moving rollers.
