29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a.) Titusville Fabricators, Inc., Bay A rebar coating area, on or about November 15th, 2019 and times prior thereto: Employees worked with and around corrosive chemicals including, but not limited to, Greenbar Touchup part B, during mixing operations of the product and rebar spray coating operations in Bay A of the facility. Employees did not have access within the immediate work area to a suitable eyewash/shower station. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a.) Titusville Fabricators, Inc., maintenance shop, on or about November 15th, 2019 and times prior thereto: Employees utilized a Central Machinery 6" bench grinder (serial #35413-1845) for grinding epoxy off of spray gun nozzles. The right side of the Central Machinery brand grinder was not equipped with a proper functioning tongue guard. The opening between the wheel and the end of the tongue guard was measured at approximately 3/8 of an inch. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.215(d)(3): The contact surface(s) of wheel(s), blotter(s) or flange(s) on grinding machine(s) were not flat and free of foreign matter: a.) Titusville Fabricators, Inc., maintenance shop, on or about November 15th, 2019 and times prior thereto: Employees utilized a Central Machinery 6" bench grinder (serial #35413-1845) for grinding epoxy off of spray gun nozzles. Employees were potentially exposed to a struck-by hazard in that, the right grinding wheel had a groove worn into the middle of the grinding wheel. The surface of the wheel on the right side of the bench grinder was not maintained in a flat condition.
