29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) On or about November 14, 2019, employee operating a manual tire inflator at station two (2) was exposed to a pinch point hazard as the operating cycle ends or after removal of hand from the two hand control. The pinch point hazard occurs as the cylinder returns to its upstroke resting position.     (b) On or about November 14, 2019, employee(s) operating a manual tire inflator at station three (3) was exposed to a pinch point hazard as the operating cycle ends or after removal of hand from the two hand control. The pinch point hazard occurs as the cylinder returns to its upstroke resting position.    (c) On or about November 14, 2019, employee(s) operating an automatic tire inflator at station one (1) was exposed to an ingoing nip point and rotating parts created by the chain drives of the automated tire inflator.  The four chains and gears continue to rotate when hands are removed from the two hand controls and tires are loaded into the inflator. This exposed employee(s) to an amputation injury.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) On or about November 14, 2019, employee direct access to emergency eye wash station from the electric forklift battery charging station was hindered.  Employees would need to navigate around material storage racks and other equipment to access the emergency eyewash station.
