29 CFR 1910.119(f)(3): Operating procedures are to be reviewed as often as necessary to assure that they reflect current operating practice including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities. The employer shall certify annually that these operating procedures are current and accurate. On or about November 4, 2019, at Total Petrochemical & Refining in Port Arthur, TX, the employer did not ensure that Operating Procedures, including operating limits in section 2.0, were reviewed as often as necessary to reflect current operating practices as a result of the changes made to the technology and equipment in the NHT/Reformer Unit.
