On or about November 19, 2019, the employer did not ensure that employees performing maintenance on the Plant 2 extrusion blow mold (EBM) machine 6B were using the energy control procedures developed while performing a high pressure air hose replacement.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  a. On or about November 19, 2019, employees were exposed to machine hazards associated with moving parts and struck by hazards during servicing and maintenance work of the Plant 2 extrusion blow mold (EBM) machine 6B. The employer did not implement energy control application steps as the machines pneumatic energy source was not isolated [per the 1910.147(d)(3) requirements]. As a result, the stored energy was not controlled by the application of a LOTO device [(d)(4)], the release of stored pressure [(d)(5)(i)], and verification of the hydraulic and pneumatic source isolation [(d)(6)] were not implemented to protect employees from machine servicing hazards.

29 CFR 1904.7(b)(4): How do I record a work-related injury or illness that results in restricted work or job transfer? When an injury or illness involves restricted work or job transfer but does not involve death or days away from work, you must record the injury or illness on the OSHA 300 Log by placing a check mark in the space for job transfer or restriction and an entry of the number of restricted or transferred days in the restricted workdays column.   Located at 9005 Smiths Mill Road North, New Albany, Ohio:    a. On the 2019 OSHA 300 log, the employer did not ensure the number of days employees were under work restrictions was accurately recorded on the log.     1.     On June 29, 2019, an employee was injured while performing a mold change that resulted in both days away from work and job restrictions. The employer did not accurately record the number of days the employee was away from work. The employer did not record the number of days the employee was on job restrictions.

You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.  Located at 9005 Smiths Mill Road North, New Albany, Ohio:    a. On the 2019 OSHA 300 log, the employer did not ensure that all injuries that resulted in days away from work or job transfer/restriction were recorded on the log.     1.     On September 26, 2019, an employee was injured while lifting boxes that resulted in days away from work and job restrictions. The employer did not record the injury on the log.
