29 CFR 1910.30(a)(3): The employer did not provide employees training in at least the following topics; the procedures to minimize fall hazards; the correct procedures for installing, inspecting, operating, maintaining, and disassembling personal fall protection systems that the employee uses; and the correct use of personal fall protection systems and equipment. On or about November 13, 2019, and at times prior thereto, the employer did not provide effective training to claims adjusters that consisted of procedures on how to recognize and minimize fall hazards while performing insurance claims operations, exposing employees to the hazards of falling from heights of 6 feet or greater. Instance: On or about November 13, 2019, an employee performing insurance claims operations was exposed to a fall hazard of approximately 14.5 feet by walking on a roof surface that could not support his weight and was damaged, exposing employees to the hazards of falling through a walking-working surfaces.
