29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.   a.  On or about November 15, 2019, at the west side of the building, the 4th scaffold section was not fully planked where two employees worked.  Employees were exposed to falls greater than 10 feet to a lower level.

29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end:   a.  On or about November 15, 2019, at the northwest side of the building, the scaffold platforms ends extended over their supports more than 12 inches.  The scaffold platforms were less than 10 feet in length, were not designed and installed so the cantilevered portions of the platforms were able to support employees without tipping, the scaffold ends were accessed to climb down from the scaffold platforms, and employees were exposed to falls greater than 10 feet to a lower level.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:   a.  On or about November 15, 2019, at the north and west sides of the building, the scaffolds were not inspected for visible defects by a competent person before three employees started working from the scaffold to lay stone.  Employees were exposed to falls greater than 10 feet to a lower level.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).    a.  On or about November 15, 2019, at the west side of the building, the 4th platform of the supported scaffold lacked guardrails and was greater than 10 feet from the ground.  Two employees were at the 4th scaffold platform to lay stone and were exposed to falls greater than 10 feet to a lower level.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.   a.  On or about November 15, 2019, at the north and west sides of the building, the scaffolds lacked toeboards where three employees worked from the scaffold to lay stone and where 1 employee worked at the ground.  Employees were exposed to struck-by hazards from materials and equipment used overhead.
