Roofers were removing/installing shingles on a 10/12 pitch roof that varied from 12-35 feet. The employees were not utilizing proper fall protection which exposed them to falls up to 35 feet.

29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection. a) Rafas Roofs, LLC, located at 6830 Macneil Drive, Dublin, OH 43017: On and preceding November 18, 2019, the employer did not provide and install an adequate fall protection system before employees began work on the roof. Roofers were removing shingles and installing weather barrier on a 10/12 pitch roof that varied from 12-35 feet. The roofers were wearing climbing harnesses with climbing ropes tied to anchor points, not attached to the harnesses. The following condition exposed employees to falls up to 35 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Rafas Roofs, LLC, located at 6830 Macneil Drive, Dublin, OH 43017: On and preceding November 18, 2019, the employer did not ensure that the portable ladder used to gain access to the second floor extended at least three feet beyond the upper landing that they were using for access. Two extension ladders used to gain access to roof section which varied between 12-16 feet were positioned 12-16 inches beyond the upper landing. The following condition exposed employees to falls up to 16 feet.
