29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Rafas Roofs, LLC located at 6575 Greenway Loop, Dublin, OH 43016: On and preceding January 28, 2020, the controlling employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. Roofers were installing shingles on a roof that varied from 10-20 feet. The employees were not utilizing proper fall protection which exposed them to falls up to 20 feet. Rafas Roofs, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1445504, citation number 1, Item 1, issued November 20, 2019 and became a final order on December 3, 2019 with respect to a workplace located at 6830 Macneil Drive, Dublin, OH 43017
