29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Production Floor, Machine 997: On, or about, November 12, 2019, an Operator's shirt was pulled into the in-running nip point of material being wound onto a finish core. The Operator could not reach either of the emergency stop devices mounted at the ends of the Rewind unit. Adequate machine safeguarding such as, but not limited to, a presence sensing device, had not been provided on the machine to passively protect employees from in-running nip points and rotating parts.
