29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  a) This violation occurred on or about November 18, 2019, at the jobsite located at 1711 West Royal Ln., building #1721, Irving, TX; where employee was using an aerial lift basket JLG, model M600JP, serial #0300204503 to access/egress the roof and was not protected from falling to the ground below.
