29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    A. On or about November 21, 2019 employees on the 3rd floor exterior balcony, to gain access to the roof were not protected by personal fall protection, guardrails or safety nets exposing them to a 22-foot, 4-inch fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about November 21, 2019 two employees installing roof tiles on a 3 story, 6:12 pitch residential dwelling were not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems, exposing employees to 42-foot 2 inch fall hazard.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.    a. On or about November 21, 2019 the extension ladder used to gain access to the roof, was not secured to the fascia or roof top to prevent accidental displacement exposing the employees to a 22-foot 4- inch fall hazard.
