29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:Note: If the employer could demonstrate that it was infeasible or created a greater hazard to use these systems, a fall protection plan meeting the requirements of paragraph (k) of 1926.502 should have been developed and implemented. a) Greenheart Companies located at 1216 W High Ave. New Philadelphia, OH 44663: On and preceding November 22, 2019, the controlling employer did not ensure that employees performing leading edge work, including the placement and securing of wood decking 15 feet above the lower level were protected from fall. No fall protection plan was in place and fall protection devices were not in place. Greenheart Companies was previously cited for a violation of this occupational safety and health standard or its equivalent 29 CFR 1926.501(b)(2)(i) which was contained in OSHA inspection number 1092086 , citation number 1, Item 1, issued October 02, 2015 and became a final order on October 13, 2015 with respect to a workplace located at 4170 Steel Points Dr., Avon, OH 44224

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) Greenheart Companies located at 1216 W High Ave. New Philadelphia, OH 44663: On and preceding November 22, 2019, the controlling employer did not ensure that employees were protected from tripping in or stepping into or through holes by covers. Throughout the process of constructing walls, setting trusses and framing in a commercial building, employees were working around floor holes located around two structural support beams on the ground level of the building. The following condition exposed employees to trips from floor holes measuring 9-24" wide and 6" deep.
