29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: (a) Front of the Pennsylvania State Police Headquarters Building - The competent person did not ensure the correct use of all components of personal fall arrest systems as evidenced by the fact that an employee used a FallTech self-retracting lifeline, model 7276WR, for tying off in an aerial lift even though FallTech did not approve this fall protection component for use in aerial lifts, exposing the employee to a fall hazard, on or about November 21, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

FallTech did not design and approve this fall protection component for use in aerial lifts, exposing the employee to a fall hazard, on or about November 21, 2019. Abatement certification required within 10 days after abatement date. The labeling and markings on the FallTech self-retracting lifeline, model 7276WR, had become illegible due to wear, and this personal fall arrest component did not get removed from service, on or about November 21, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
