On or about November 20, 2019, at a trench greater than 4 feet in depth, the employer did not ensure that the ladder to exit the trench was placed within a trench box and this exposes employees to cave-in hazards.

On or about November 20, 2019, at the trench, the employer did not ensure that the stability of a street lamp was not endangered by excavation operations. The street lamp was not supported or braced which exposes employees to struck-by and cave-in hazards.
