29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  Building #26. On or about September 21, 2021, an employees working from an aerial lift(s) were not wearing a safety harness with lanyard in the following instances:  (a) Employee installing building wrap on east side of building #26 (b) Employees installing plywood on balcony on the north side of building #26   The employees were exposed to the hazard of falls.
