29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Items 1a/b for employees exposed to fall hazards and lack of fall protection and training.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about November 22, 2019, employees were exposed to a fall of approximately 20 feet while performing framing activities on a residential home located at 36650 Amalfi, North Ridgeville, Ohio 44039.  The employees were working on the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about November 22, 2019, employees were exposed to a fall of approximately 20 feet while performing framing activities on a residential home located at 36650 Amalfi, North Ridgeville, Ohio 44039.  The employees were working on the roof and the employer did not ensure that they were trained on how to recognize and minimize fall hazards.
