29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: (a) Jobsite: On or about November 25, 2019, an employee climbed down the spreader bard to two trench boxes to enter into the approximately 17 foot deep excavation. The bottom spreader bar was about 6 feet from the bottom of the excavation.
