29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing. On or about November 11, 2019, a Material Handler operating a powered industrial vehicle did not slow down when entering aisle AJ and struck an employee. Material stacked on the corner of the aisle obstructed the forklift operator's vison, increasing the Material Handler's stopping distance, resulting in the operator hitting another employee.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. On or about November 26, 2019, the employer was unable to produce a workplace hazard assessment that identified the person who certified the evaluation of employees engaged in job tasks such as operating the hot glue systems, Powered Industrial Vehicles, and handling chemicals.
