29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  Worksite located at 8120 N Hickory St, KC MO -- Employees were performing roofing duties when some employees were working on the ground and were exposed to struck by hazards from overhead of excess of 25 feet.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At worksite located at 8120 N Hickory St, KC MO -- Employees were not wearing any eye protection while performing roofing duties using pneumatic nail guns. Employees were exposed to struck-by hazards.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage.  Worksite located at 8120 N Hickory St, KC MO -- The employer did not inspect the flexible cord for damage. Employees were using a flexible cord to power portable hand tools, and had a noticeable tear in the insulation that exposed the internal wires. Employees were exposed to electrical shock and burn hazards. This was documented on or about Monday, November 25, 2019.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Worksite located at 8120 N Hickory St, KC MO -- Employees were roofing an apartment building, commercial property, with no fall protection, guardrails or safety net systems. Employees were exposed to fall hazards. This was documented on or about Monday, November 25, 2019.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Worksite located at 8120 N Hickory St, KC MO -- Employees were using portable aluminum extension ladders to gain access to the roof of the commercial apartment building. The ladders were not extended 3 feet above the landing surface, in this case, the roof. Employees were exposed to fall hazards.
