29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage. Worksite located at 8120 N Hickory St, KC MO -- The employer did not inspect the flexible cord for damage. Employees were using a flexible cord to power portable hand tools, and had a noticeable tear in the insulation that exposed the internal wires. Employees were exposed to electrical shock and burn hazards. This was documented on or about Monday, November 25, 2019.
