The employee was observed without eye protection while using a pneumatic nail gun during roofing activities.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: At a worksite located at 8110 North Hickory Street, Kansas City, Missouri: Employees were roofing an apartment building and were exposed to fall hazards. The employees were wearing harnesses which were not connected to their lifelines. Roofers were exposed to a fall distance of approximately 25 feet eave to ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Worksite located at 8110 North Hickory Street, Kansas City, Missouri: Employees were exposed to fall hazards. Employees were using a portable extension ladder to gain access to a third floor balcony of the apartment building. The ladder was not extended 3 feet above the third floor balcony.
