29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about November 25, 2019, the employer failed to utilize their lockout procedures when doing servicing and/or maintenance on equipment including, but not limited to, the Piranha (serial #20316 and 20292), the Accupress (serial 8779) and the Bliss Press (no serial number but only one in facility), when employees were changing dies and punch heads.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolate:      On or about November 25, 2019, the employer failed to ensure that employees who are tasked with changing dies, punch heads and performing other services on presses, to include but not limited to the Piranha (serial #20316 and 20292), the Accupress (serial 8779) and the Bliss Press (no serial number but only one in facility), were trained as authorized employees.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about November 25, 2019, the employer failed to ensure that the rear of the presses, to include but not limited to the Piranha (serial #20316 and 20292) and the Accupress (serial 8779), were guarded so as to prevent employees from coming into contact with moving parts and the point of operation.  Employees routinely have to enter the back of the presses to perform tasks such as but not limited to die changes.

29 CFR 1910.217(b)(7)(v)(a): Each hand control was not protected against unintended operation and arranged by design, construction, and/or separation so that the concurrent use of both hands was required to trip the press:    On or about November 25, 2019, the employer failed to ensure that the two hand controls being used to operate the Bliss Mechanical Power Press #3, were designed so as to protect from any unintentional contact.  The two hand controls had no guard or shroud over or around them.

29 CFR 1910.23(b)(8): The employer did not ensure ladders were used only for the purposes for which they were designed:  On or about November 25, 2019, the employer failed to ensure that employees who were using portable A frame type ladders which allowed them to access various parts of the tankers they are working on, was used as its designed intent.  Employees were using the ladders in the closed position and leaning them up against the tankers and then climbed on them to perform various tasks.
