Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system alone: a) On or about 11/19/19, at a 2-story house located at 130 Prospect Ave., Buffalo, NY: Two employees engaged in roofing work on the low slope porch roof on the front of the house were exposed to a fall of approx. 12 ft. because no fall protection system was utilized. The employees were not wearing a body harness and no other fall protection system was utilized. The pitch of the porch roof was approx. 3 in 12. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 11/19/19, at a 2-story house located at 130 Prospect Ave., Buffalo, NY: One employee doing roofing work on the steep slope high roof on northwest side of the house was exposed to a fall of approx. 18 ft. because no fall protection system was utilized. The employee was not wearing a body harness and no other fall protection system was utilized. The pitch of that roof was approx. 8 in 12. b) On or about 11/19/19, at a 2-story house located at 130 Prospect Ave., Buffalo, NY: Two employees engaged in roofing work on the steep slope garage roof on the left side of the house were exposed to a fall of approx. 10 to 14 ft. because no fall protection system was utilized. The employees were not wearing a body harness and no other fall protection system was utilized. The pitch of the garage roof was approx. 8 in 12. NO ABATEMENT CERTIFICATION REQUIRED

The siderails of that extension ladder were approx. 30 inches above the roof landing. NO ABATEMENT CERTIFICATION REQUIRED
