OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to an amputation hazard.     a) Orwigsburg Facility, Production Area - Employees were required to operate a Philadelphia Tramrail Company 3400HD baler (older baler) while the proximity switch on the safety gate was defective, exposing employees to a possible amputation injury. Employees could still operate the baler, while the safety gate was in the open position on or about November 27, 2019.    b) Orwigsburg Facility, Production Area - Employees were required to operate a Philadelphia Tramrail Company 3400HD baler (older baler) while two holes located on the safety gate exposed employees to a possible amputation injury on or about November 27, 2019 and December 6, 2019. The first hole measured 2.5''x4.5" and the second hole measured 3"x3.5".
