29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A)     Next Generation Roofing, conducting work at 11043 Bay Trace, Perrysburg, OH 43551: On or about November 26, 2019, the employer did not ensure that employees were protected from head injury hazards by protective hard hats while working below employees who were conducting roofing work above. Employees were exposed to potential impact injuries caused by falling materials and/or equipment.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   A)     Next Generation Roofing, conducting work at 11043 Bay Trace, Perrysburg, OH 43551: On or about November 26, 2019, the employer did not ensure that employees were adequately trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while conducting roofing activities atop the multiple levels of the two story residential structure. Employees did not use fall protection and were exposed to the fall hazards of approximately 10 to 19 feet from the multiple levels of the structure to the ground below.

Employees were descending the extension ladder with their back toward the ladder and were exposed to approximately 19 feet fall hazard from the ladder to the ground below.

Employees were descending the extension ladder with their back toward the ladder and were exposed to approximately 19 feet fall hazard from the ladder to the ground below.
