29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. A) Next Generation Roofing, conducting work at 5143 Comstock Drive, Sylvania, OH 43560: On or about September 2, 2020, the employer did not ensure that portable ladders, used to access the roof of the one story residential structure, were extended at least three feet above the landing surface. Employees were exposed to approximately 10 feet fall hazard when using the ladders to access the roof in order to conduct construction activities.

Employees were exposed to potential impact injuries caused by falling materials and/or equipment. Next Generation Roofing was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.100(a)), which was contained in OSHA Inspection # 1448090, Citation number 1, Item number 1, and was affirmed as a final order on 03/10/2020, with respect to a workplace located at 11043 Bay Trace, Perrysburg, OH 43551.
