29 CFR 1910.106(d)(3)(ii): Flammable liquid storage cabinet was not labeled in conspicuous lettering, "Flammable - Keep Fire Away":  Building 00879: External Rear: A Flammable liquid storage cabinet with no label using conspicuous lettering of "Flammable - Keep Fire Away" was used by employees to store flammable materials. Employees exposed to explosive and fire hazards.

29 CFR 1910.106(d)(5)(iii): Storage of flammable liquids in office occupancies, except that which is required for maintenance and operation of building and operation of equipment, was not prohibited. Such storage was not kept in closed metal containers stored in a storage cabinet or in safety cans or in an inside storage room not having a door that opens into that portion of the building used by the public.  Building 00879: Inside Break Room, Supply Room, and Weapon Control Lockers: Employees used and stored So-Sure Aerosol Fluorescent Lacquer, Birchwood Casey Gun Scrubber Solvent, and Rem Oil Lubricant Aerosol inside the office. Employer did not prohibit the storage of these chemicals from inside the office and require chemicals to be stored in the outside flammable storage cabinet.  Employees were exposed to explosive and fire hazards.

29 CFR 1910.132(e): The employer did not remove from service defective or damaged personal protective equipment.  Sentry buildings: Employees used high visibility coats during evening and early morning plus at times of rain with low visibility. Coats were damaged and torn which prevented reflective materials from being visible. The employer did not remove the high visibility coats from service. Employees were exposed to struck-by hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  Interior Building, Employee Breakroom, and Sentry Houses: Employees used Defense Technology MK-4 Spray for sentry duties, Purell Hand Sanitizer as hand disinfectant at guard houses, and ZEP Heavy Duty Floor Stripper concentrate on hallways inside the building to remove scuffs and old wax build-up.  The employer had not provided training specific to the health and safety hazards of these chemicals. Employees were exposed to dermal, respiratory and eye hazards.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  Sentry Gate House 11020: Employees voluntarily used 3M 8511 N95 filtering face piece respirators conducting traffic management during high wind conditions and stored the respirator outside a closed container and on top of a window shelf. Employer did not ensure that respirators were stored to protect from damage to facepiece or exhalation valve. Employees were exposed to inhalation and dermal hazards.
